ADHD medication tracker (young people)

A simple way to track how ADHD medication is working, for children, teenagers, and young adults. It adds up the score for you, shows the response band, and keeps a record of past reviews. It stays on this device.

When to use it: fill it in for each medication review, rather than on a fixed schedule. Your prescriber may ask for it more often while the dose is being adjusted, and less often once things are settled.
