This works for any ADHD medication, whether a stimulant or a non-stimulant such as atomoxetine, bupropion, clonidine, or an SSRI. Fill in what is relevant to you, and leave the rest blank.
For example: feeling jittery or wired, dizziness, sweating, a change in weight, stomach upset, drowsiness, or feeling low or irritable. Note anything you want to raise.
If you ever have chest pain, a racing or irregular heartbeat that does not settle, fainting, a marked change in mood, or any thoughts of harming yourself, do not just note it here. Contact your doctor, or your nearest hospital if it is urgent.
The spreadsheet is for your doctor. The backup file is for you: it can be restored here to move your log to another device or recover it if this one is wiped.
